Job Summary
Conduct comprehensive in-home assessments and care plan development for frail, homebound seniors to support their safety and independence. Manage complex client cases including APS referrals, ensuring timely DFTA assessments and monthly reporting. Provide crisis intervention for medical emergencies, utility issues, and disaster response, while offering emergency coverage for Home Delivered Meals. Maintain accurate case notes and follow up on service referrals. Requires a Master of Social Work degree and experience with older adult entitlements.
Required Qualifications
- Master of Social Work Degree from an accredited college or university
- One year of experience in human services field
- Knowledge of entitlements for older adults
- Accredited with Supervision in Field Instruction within 6 months of employment
- Capable of assisting with more complex client issues
- Able to travel to multiple locations within the five boroughs as needed
- Frequently lifts and/or moves up to 10 pounds
