Social Worker II - Case Management

Provide necessary support for patients referred to your discipline based on health status, diagnosis, and specific needs. Collaboratively work with interdisciplinary teams to ensure completion of patient-specific care plans and post-acute needs. Effectively collaborate with community resources to align patients with available referrals and governmental assessments. Provide counseling services to patients and families when appropriate, while assisting various populations including APS, CPS, and DSS based on identified safety needs. Handle customer complaints promptly using proper escalation procedures when problems are encountered. Maintain a Master's degree and Licensed Master Social Work certification with 1-3 years of related experience.
