Job Summary
Social Worker II at West Side Federation for Senior and Supported Housing in New York. Provides autonomous social work services to residents, assisting with medical/mental health services, entitlements, home care, and care coordination; supports social relationships, community participation, budgeting, and housing stability. Delivers crisis intervention, individual and group counseling, discharge planning, and follow-up with service providers; maintains case records and adheres to confidentiality. Engages in intake processes, participates in supervision, and collaborates with staff. The employer supports MSW licensing pathways, provides licensing exam coverage, CEU funding, and weekly clinical supervision by an LCSW. Bilingual English/Spanish required; experience with older adults and mental illness/substance use preferred.
Required Qualifications
- MSW preferred, or MA in human services field with a minimum of two years social work experience
- Fluency in written and oral English and Spanish required
