Provide assessment, referral, education, service planning, and care coordination services to CC clients, focusing on increasing self-management and adherence to medical and behavioral health treatment plans. Collaborate with an interdisciplinary team including CrescentCare and community providers while maintaining a close working relationship with a direct supervisor. Participate in active supervision and remain receptive to feedback on professional development to support high-quality service provision.
