Job Summary
Social Worker I provides autonomous social work services to residents in senior housing, developing 1:1 relationships to help residents establish and maintain safe, independent lifestyles. Responsibilities include assisting residents with obtaining and maintaining medical and mental health services, entitlements, home care, budgeting and income maintenance; delivering crisis intervention, individual counseling, and group work; coordinating intake, discharge planning, and referrals; maintaining thorough case records; and participating in supervision and staff meetings. The role supports licensing progression (MSW to LMSW, with supervision and CEU support) and requires MSW/MA in human services with at least two years of social work experience, with fluency in English and preferred experience with older adults and mental illness/substance use.
