Job Summary
Coordinate screening, assessment, and discharge planning for high-risk patients and families with complex psychosocial needs across five Northwestern Medicine Cancer Centers. Provide crisis-oriented psychotherapy, administer psychosocial functioning tests, and develop evidence-based treatment plans while addressing social, emotional, and cultural determinants of health. Collaborate with care teams, post-acute providers, and community resources to ensure seamless discharges and identify at-risk populations. Supervise lower-level social workers, students, or interns and educate staff on socio-economic and emotional issues impacting care delivery. M-F coverage with 8a-4p hours, two shifts per month off, and a requirement for one year of acute hospital social work experience.
Required Qualifications
- One full year experience and/or internship in a medical social work role in and acute hospital setting or agency
- Experience with maternal/child populations
- Licensed Social Worker (LSW)
- Must complete background check authorization and disclosure form
- Must adhere to organization's Code of Ethics and Corporate Compliance Program
- Must comply with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards
- Must be available for covering needs at the five NM Cancer Centers in Orland Park, Oak Brook, Warrenville, Geneva, and DeKalb
- Must work M-F no nights, weekends, or holidays
- Must assist with covering scheduled days off and leaves as applicable at least two shifts per month
- Must work typical general hours of 8a - 4:30p
Desired Qualifications
- Two years' experience in an acute hospital setting
- Experience in a level III nursery for maternal child area
- Licensed Clinical Social Worker (LCSW)
- ACSW (Academy of Certified Social Workers)
