Job Summary
Social Worker – Hospital Discharge role coordinating and managing discharge for adults with complex needs within the London Borough of Richmond upon Thames. Utilises the Discharge to Assess framework, conducts Care Act 2014 and Mental Capacity Act assessments, and develops strengths-based, person-centred care plans to promote independence and safe home/community transitions. Collaborates with health professionals, therapy teams, housing providers, and voluntary sector partners; performs safeguarding in line with statutory guidance; ensures records compliance with Care Act, MCA, and DoLS. Includes responsibilities around timely, safe, and person-centred hospital discharges aligned with a Home First approach.
Required Qualifications
- Social Work England registration
- Eligible to work in the UK
- Full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
