Job Summary
Conduct thorough psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers while providing ongoing emotional support and counseling. Identify and connect families with community resources, including financial assistance, and guide them through advanced directives and facility placement. Maintain accurate clinical documentation at residences and in agency records, actively contributing to interdisciplinary team meetings to shape holistic care plans. Participate in on-call duties for evenings and weekends as needed while managing caseloads in compliance with local, state, and federal regulations. Travel locally to serve coverage territories in Ottawa, Streator, Dwight, Kankakee, and surrounding areas.
Required Qualifications
- Bachelor's degree in Social Work
- Master's degree from school accredited by the Council on Social Work Education required in states as specified
- Licensure and/or certification as required by the State
- Minimum of 2+ years social service experience in a health care setting or social services agency
- Valid driver's license
- Willingness to travel locally for patient visits
