Job Summary
Prepare social histories, develop care plans, and provide social casework to patients and families facing social functioning challenges. Assess psychosocial status, facilitate referrals to community agencies, and interpret medical situations to referring professionals. Participate in case conferences, conduct discharge planning, and maintain clinical progress notes while managing financial assistance assessments. Deliver short-term individual counseling and crisis intervention as needed. Requires a Master's degree in social work, one year of hospice experience, and CPR certification. Required Qualifications
