Job Summary
Conduct comprehensive psychosocial assessments for patients and families, developing individualized plans of care in collaboration with the interdisciplinary team. Deliver direct services including short-term counseling, crisis intervention, advance directive preparation, and bereavement support while identifying community resources and assessing caregiver capacity. Coordinate admission visits, discharge planning, and transfer logistics, reporting changes in patient condition to physicians and facilitating communication between families and clinical staff. Maintain clinical documentation, participate in quality improvement reviews, and lead bereavement groups as needed.
Required Qualifications
- at least 2 years of master's level experience in an interdisciplinary health care setting
- at least a MSW from a Social Work Education accredited school
- medical licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, if applicable
- consent to drug and/or alcohol testing after a conditional offer of employment is made
- ongoing compliance with the Drug-Free Workplace Policy
- ability to demonstrate superlative oral and written communication skills
- ability to organize and manage projects within defined timeframe and budget
- ability to work both independently and as part of a team
- excellent interpersonal and collaborative skills
- organizational skills with ability to delegate and follow up
- ability to manage multiple tasks and stressful situations with a positive attitude
- strong work ethic and attention to detail
- skills in psychosocial assessment, crisis intervention and counseling
- knowledge of hospice-appropriate theories, e.g. general and family systems, object relations, transpersonal, and cognitive-behavioral, etc.
- ability to be an advocate for patient/family within the structure of the team, agency, medical setting, and community
- ability to conduct effective community education that enhances understanding of special needs of terminally ill
- sensitivity to issues of loss and grief
