Job Summary
Assess psychosocial status of patients and families regarding terminal illness and environment, communicating findings to the registered nurse and interdisciplinary group. Conduct social evaluations covering family dynamics, caregiver abilities, and suicide risks to plan interventions, while counseling on stress and coping difficulties. Provide information and referral services for practical and environmental needs, acting as a liaison between patients, families, and community agencies. Maintain clinical records, educate on advanced directives, and assist in funeral and financial planning. Participate in interdisciplinary meetings, quality assessment teams, and individualized care plan development. Requires a master's degree in social work with two years of supervised healthcare experience, flexibility for travel, and willingness to maintain comprehensive information systems knowledge. Full-time role with tuition reimbursement, PTO, medical/dental/vision insurance, 401(k), and mileage reimbursement.
Required Qualifications
- Master's degree in social work from an accredited university
- Undergraduate B.A. Degree in Social Work
- Minimum of two (2) years of documented supervised experience in health care
- Hospice experience
- Understanding of hospice philosophy
- Understanding of issues of death/dying
- Flexibility in work hours
- Ability to travel throughout the assigned Bristol Hospice service area
- Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
