Job Summary
Counsel patients on illness progression, participate in treatment plan development, and arrange in-home care personnel and equipment. Educate families regarding diagnosed conditions, assist in care decision-making, and provide grief counseling to reduce stress. Coordinate with insurance companies, community agencies, and financial programs to determine eligibility and complete necessary paperwork. Evaluate mental, emotional, and physical patient status while collaborating with doctors, nurses, and healthcare representatives. Maintain detailed records and adhere to federal, state, and local healthcare standards. Requires active state licensure, strong communication skills, and basic technology proficiency. No weekend or on-call commitment. Located in Northern Colorado.
