Job Summary
Assess and evaluate homeless clients and families referred by NYC Department of Homeless Services, developing individual living plans with therapeutic support. Provide clinical supervision to Case Managers through case analysis, service planning, and supervisory processes while conducting clinical case conferences to address unmet needs. Coordinate mental health referrals and psychiatric care, develop a network of social service providers, and facilitate group meetings. Monitor resident records for compliance, review progress notes, and function independently during facility coverage, including crisis intervention via on-call rotation. Participate in weekly supervision, staff meetings, and trainings to enhance clinical skills.
Required Qualifications
- Masters Degree in Social Work from an accredited university
- Three years of experience, working with the homeless population
- Demonstrated sensitivity and experience with and/or knowledge of homeless families including individuals whom identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, the elderly and individuals with substance abuse and/or mental health issues
- Familiarity with DHS regulations and DHS Cares Reporting
- Clinical experience to include the ability to make psychosocial assessments and develop and implement individual living plans
