Job Summary
Provide psychosocial assessment and treatment planning for home health patients, coordinating with a multidisciplinary care team to develop and implement effective plans that optimize quality of life for patients and families. Develop and maintain therapeutic relationships, educate patients and families, and ensure cost-effective, high-quality care in the home health setting. Requires a Master of Social Work (MSW) and licensure (LSW or LCSW) with Illinois licensure; 1-2 years of clinical post-licensure experience preferred. Includes travel within designated Chicago SE communities; weekdays 8-5 with weekend/holiday rotations; valid IL driver’s license required; base compensation commensurate with range $30.70-$46.05/hour, plus benefits and sign-on options.
Required Qualifications
- Masters of Social Work services from an accredited school with licensure or licensed social worker (LSW) or licensed clinical social worker (LCSW)
- 1-2 years of experience in a clinical setting post licensure preferred
- A valid driver’s license issued by the Division of Motor Vehicles
