Job Summary
Provide full social work services to patients and families at the Holland Orthopaedic and Arthritic Centre, including psycho-social assessments, counselling, discharge planning, crisis intervention, and linkage to community resources. Conduct team consultations while managing multiple tasks and setting priorities in a fast-paced environment. Maintain exceptional communication skills and demonstrate strong negotiation and conflict resolution abilities. Apply in-depth knowledge of the Sunnybrook Discharge Policy and issues of capacity and consent to support patient care. This role supports student education and requires a commitment to the team.
Required Qualifications
- Masters Degree in Social Work
- Member of the Ontario College of Social Workers and Social Service Workers
- Minimum two (2) years of experience working in a Hospital setting
- Exceptional communication skills
- Familiarity with issues of capacity and consent
- In-depth understanding of the Sunnybrook Discharge Policy and its application
- Strong negotiation and conflict resolution skills
- Ability to be a team player as well as self-directed
- Ability to manage multiple tasks and set priorities in a fast-paced environment
- Commitment to student education
