Job Summary
Licensed Social Worker needed to perform visits and coordinate care for patients in their residence as part of an interdisciplinary team. Must be licensed in Idaho as an LSW, LMSW, or LCSW and hold BLS CPR certification. Home Health and Hospice experience is preferred but not required; position involves working with patients and families in settings serving those recovering from illness, injury, surgery, or living with chronic disability, including end-of-life care in hospice. The role emphasizes collaboration, comprehensive patient care coordination, and adherence to agency, state, and federal policies.
Required Qualifications
- Licensed in the state of Idaho as a Licensed Social Worker (LSW), Licensed Master’s Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW)
- BLS/CPR certification
- Home Health and Hospice experience preferred, but not required
- Meet all licensing requirements related to your clinical license
