Job Summary
Social Worker – SUD/Behavioral Health responsible for assessing, planning, coordinating, and providing services to residents with substance use disorders, mental health conditions, and behavioral challenges. Responsibilities include psychosocial assessments, development and implementation of individualized care plans with the interdisciplinary team, counseling and crisis intervention, harm reduction and recovery-oriented approaches, MAT coordination when applicable, discharge planning, community referrals, collaboration with physicians and care teams, and compliant documentation per CMS/state regulations. Education and ongoing counseling for residents and families, and participation in quality improvement activities (QAPI) are also included.
Required Qualifications
- Bachelor's Degree in Social Work (BSW)
- Current state licensure (LSWAIC, LASW, LICSW, or equivalent depending on state requirements)
- Minimum one (1) to two (2) years of experience in behavioral health, substance use treatment, healthcare, or long-term care settings
