Job Summary
Social Worker opportunity at Heartland Living & Rehabilitation in Greensboro, NC. The facility provides long-term care and rehabilitation services with social and recreational activities. Must have a Bachelor's degree in Social Work or related human services field, at least two years in long-term care, strong verbal/written communication, experience with MDS/CAA/CP/BIMS/PHQ9, competency in electronic charting (PointClickCare preferred), proficiency with Outlook and Microsoft Office, and a demonstrated ability to work in a team. Required Qualifications
- Bachelor's degree in Social Work or other human services area such as sociology, special education, rehabilitation counseling or psychology
- At least two years of experience in long term care
- Excellent verbal and written communication skills
- Experience in completion of MDS, CAA and CP, BIMS and PHQ9
- Strong organizational skills as evidenced by the ability to multi-task
- Have compassion for the elderly and for their families
- Proven ability to work in a team environment
- Demonstrated investigation skills related to customer complaints and other resident concerns
- Competency in electronic charting, PointClickCare preferred
- Strong working knowledge of Outlook, Microsoft Word, Microsoft Excel
- Background check and pre-employment drug screen required.
