Social Worker

Assess hospice patients to identify psychosocial, grief, financial, environmental, and community needs through documentation and clinical records. Provide individual counseling, crisis intervention, and assistance with advance directives, funeral planning, and legal transfers. Facilitate community resources based on patient and family needs while accepting clinical assignments consistent with education and competence. Perform on-call responsibilities to provide service to patients and families as assigned. Submit all documentation timely and ensure compliance with local, state, and federal laws and Medicare regulations. Requires a Master's degree in social work, current state license, and one year of clinical experience. Full-time role with salary, comprehensive benefits, and travel requirements.
