Social Worker

Assess hospice patients to identify psychosocial, grief, financial, environmental, and community needs through documentation, clinical records, and team minutes. Provide individual counseling, crisis intervention, and assistance with advance directives, funeral planning, and legal transfers. Facilitate community resources based on patient and family needs while accepting clinical assignments consistent with education and competence. Perform on-call responsibilities and ensure timely submission of all documentation. Maintain compliance with local, state, federal laws, Medicare regulations, and personnel policies. Requires a Master's degree in social work, current state license, one year of clinical experience, and reliable transportation. Includes travel requirements.
