Job Summary
Assess hospice patients to identify psychosocial, grief, financial, environmental, and community needs through documentation and clinical records. Provide individual counseling, crisis intervention, and assistance with advance directives, funeral planning, and legal/fiscal transfers. Facilitate community resources and accept clinical assignments consistent with education and competence. Perform on-call responsibilities and ensure compliance with local, state, federal laws, Medicare regulations, and personnel policies. Submit all documentation timely. Requires a Master's degree in social work, current state license, one year of clinical experience, and reliable transportation with auto liability insurance. Full-time role with competitive pay, benefits including 401k matching and tuition reimbursement, and mileage/car allowance. Travel required.
Required Qualifications
- Bachelors or Master's degree in social work from a State accredited program
- Current State license required in accordance with State regulations
- Current Driver's License
- reliable transportation
- provide ongoing valid and current auto liability insurance
- One year of experience as a Social Worker in a clinical care setting
