Social Worker-Healthy Start FT, Day

Promotes optimal social functioning of participants in Prisma Healthy Start by providing direct and indirect social work services, supervising paraprofessionals, and developing and auditing service and treatment plans to ensure Medicaid-billable, quality care. Recruits participants, coordinates care with an interdisciplinary team, conducts crisis management and follow-up assessments of psychosocial and financial needs, educates communities on prenatal and child healthcare, and performs other duties as assigned to support maternal and child health. Requires LMSW or LPHA licensure and at least three years of social work experience; position involves supervision of paraprofessionals, program development, case management, and collaboration across healthcare and community resources. This is a day shift, full-time role based in Greenville Memorial Hospital and affiliated Prisma Health locations in South Carolina.
