Social Worker - Health Homes Care Management - Bilingual Spanish

Job Summary

Provide client-centered care management services including outreach, engagement, comprehensive intake assessments, and individualized care plan development. Coordinate medical, substance use, and mental health services while advocating for optimal health outcomes and reducing emergency department usage. Conduct clinical case work interventions using motivational interviewing, document all activities in the program platform, and ensure confidential information is protected. Independently travel between multiple off-site locations to assist clients in meeting care plan goals and accompany them to provider appointments. Participate in ongoing case conferences with the client's care team and contribute to department quality improvement efforts. Bilingual English/Spanish required with a Master's degree in social work and at least two years of healthcare experience.

Required Qualifications

  • Master's degree in social work from CSWE accredited school
  • Require LMSW or within 6 months of employment
  • Bilingual English / Spanish
  • Proficient computer skills
  • Computer operations
  • At least TWO years experience in a health care setting
  • HIV/AIDS, alcohol/substance use and/or mental health program experience
  • Valid drivers license
  • Access to personal car for work related travel
  • Well-developed and effective interpersonal skills
  • Ability to communicate effectively verbally and in writing
  • Confidence in actions and good judgment
  • Leadership ability and willingness to assume authority and accountability for the direction and supervision of others
  • Ability to plan, organize, develop and implement goals, objectives, as well as policies and procedures necessary for quality care
  • Ability to embrace and adapt to change
  • Ability to recognize problems, approach them in an objective manner, reach appropriate solutions, implement them, and evaluate for effectiveness

Desired Qualifications

  • CASAC or Qualified Health Professional

Additional Requirements

  • Must obtain LMSW within 6 months of employment
