Job Summary
Conduct comprehensive biopsychosocial assessments, develop tailored plans for social determinants of health, and perform crisis intervention, suicide risk assessments, and mental health evaluations. Facilitate discharge planning by connecting veterans to community resources such as home health care, rehabilitation, and housing. Lead family meetings and goals-of-care discussions, document care plans and safety measures in the medical record, and identify instances of abuse or neglect. Assist veterans with advance directives, petitions for inpatient psychiatric care, and accessing benefits. Requires current, full, and unrestricted LCSW licensure with at least two years of recent experience. Operates 24/7 with rotating shifts including federal holiday coverage. Required Qualifications
- Licensed Clinical Social Worker (LCSW)
- Masters in Social Work (MSW) from an accredited institution
- Current, full, and unrestricted LCSW licensure in any U.S. state or territory
- Minimum of two years of recent experience in social work, preferably within healthcare or veteran services
- Proficiency in English (spoken, written, and read)
- Commitment to VA standards of professionalism, decorum, and customer service
- Ability to lift and carry up to 50-100 lbs
- Prolonged standing, walking, bending, kneeling, and reaching during patient interactions and treatments
