Job Summary
Grade 1 or Grade 2 Social Worker based in Ballarat Base Hospital (Grampians Health). Provides care across chronic illness, cancer care, ICU, care communities and inpatient settings. Key duties include psychosocial assessment, complex discharge planning, and collaboration with a multidisciplinary team to ensure smooth transitions through the health care system. Requires strong organisational skills, sound clinical reasoning, and competencies in psychosocial assessment, intervention planning, and team-based practice.
