Social Worker (Full-Time)– Rochester, NY

Provide social work services to patients in the field and conduct assessments to develop care plans. Collaborate with interdisciplinary teams to support patient care, maintain accurate documentation, and ensure regulatory and company standards compliance. Qualifications include BSW or MSW (required) with experience working with clients with traumatic brain injury (TBI) and developmental delays. Must have strong interpersonal and communication skills, ability to work independently, and manage field-based responsibilities. Schedule is Monday–Friday, 9:00 AM–5:00 PM; field work required.
