Social Worker- Full Time (LMSW, LCSW)

Develops and oversees an individualized plan of care for assigned patients and families, addressing psychosocial, financial, and emotional needs. Provides coverage for inpatient/outpatient Hospice services as needed, liaises with community resources to obtain support, mentors interns/students, and may facilitate bereavement programs. Works within interdisciplinary teams and adheres to standard practices and licensure requirements; readiness to work outside standard hours and travel as needed. Requires strong communication, organizational, and computer skills, and a current Arkansas social work license (LMSW/LCSW preferred).
