Job Summary
The Social Worker at Jefferson Hospital ED will collaborate with all disciplines to assess, plan, implement, and evaluate psychosocial factors affecting patients’ illnesses and continuity of care. Key responsibilities include documenting psychosocial assessments, providing crisis intervention and counseling, and engaging in the discharge process to identify post-acute resources. The role requires a Master's degree in Social Work and may involve compliance with ethical and legal standards as set forth in HIPAA and company policies.
Required Qualifications
- Minimum Master’s degree in Social Work
- Older Adults Protective Services Act (OAPSA) clearance may be required depending on location
- Act 34 Criminal Background Clearance Certificate
- Act 33 Child Abuse Clearance Certificate
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
Desired Qualifications
- Preferred LSW or LCSW eligibility depending upon facility requirements
- 2 years’ experience in a hospital/multidisciplinary health care setting
- Clinical or hospital-based social work experience
Additional Requirements
- Current employees may be exempted from having a Master’s degree in Social Work
