Job Summary
Conduct discharge planning, case management, and psychosocial support for patients and families within the hospital setting. Complete psychosocial assessments and interventions to assist patients in coping with hospitalization, illness, diagnosis, and life situations. Provide support and education to families regarding post-hospitalization care. Offer advice and guidance for non-medical barriers to care that may hinder recovery or delay discharge. Serve as a resource for continuity of care options after discharge. Requires Master of Social Work degree and LCSW or CSW licensure.
Required Qualifications
- Master of Social Work from a school of social work accredited by the Council on Social Work Education", "LCSW or CSW
