Social Worker (FT, Days) Monterey Park Hospital

Provide direct social work services and counseling to patients and families while delivering psychosocial and diagnostic impressions on all seen patients. Evaluate social and family information to determine treatment plans, consult with physicians regarding illness impacts, and maintain continuity of care through follow-ups with referred patients. Assist in crisis casework for Emergency Department referrals, coordinate family conferences to discuss goals and progress, and manage relationships with community agencies. Perform management tasks including time planning, reporting, and statistics as required by the Utilization Review Department. Maintain close working relationships with appropriate community agencies and participate in hospital in-service programs.
