Social Worker FT, Day

Conduct direct and indirect services to promote optimal social functioning and address emotional problems interfering with healthcare treatment. Supervise Resource Mothers, develop Service Plans, and recruit participants for the Prisma Healthy Start Program. Review visitation forms, audit quality assurance activities, and manage crisis situations with paraprofessionals. Coordinate care with the multi-disciplinary team, assess psycho-social needs, and educate community groups on prenatal and parenting skills. Perform chart audits to ensure Medicaid compliance and interpret clinical data for recommendations. Works within an interdisciplinary team to facilitate efficient use of community resources.
