Job Summary
Social Worker to support young people and adults as they transition from children’s services into adult care. Conduct comprehensive assessments and create tailored transition plans; provide guidance and advocacy to young people and families; collaborate with multidisciplinary teams in education, health, and social care; ensure safeguarding and comply with care legislation; maintain accurate records and provide detailed reports; promote independence, inclusion, and person-centered care.
Required Qualifications
- Hold a social work degree
- Right to work in the UK and hold clean UK driving license
- Registered with SWE
- Post qualified experience within children and family services
