Job Summary
Provide psychosocial support and social work services to hospice patients and their families in inpatient, home care, and nursing home settings across pre, post, and bereavement phases. Develop social work plans of care, goals, and discharge recommendations for direct admissions and facility residents while serving as a liaison between hospice staff and inpatient or nursing home teams. This role requires New York State MSW licensure, a driver's license, and at least one year of healthcare social work experience. Compensation includes fee-for-service rates ranging from $43 to $69 per visit depending on specialty and setting.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Valid NYS Drivers license and car
- Ability to work within an interdisciplinary team environment
- Ability to input clinical data proficiently
Desired Qualifications
- Knowledge of Word and excel preferred
