Job Summary
Provide psychosocial support and social work services to hospice patients and their families in inpatient, home care, and nursing home settings across pre, during, and post-bereavement phases. Develop comprehensive social work plans of care, goals, and discharge recommendations based on patient and family needs for direct admissions. Serve as a liaison between hospice staff and inpatient or nursing home personnel while functioning as a member of the hospice interdisciplinary team. Input clinical data proficiently and work within an environment that encourages professional autonomy and minimal supervision. Requires New York State MSW licensure, a valid driver's license, and at least one year of healthcare social work experience.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Valid NYS Drivers license and car
- Ability to work within an interdisciplinary team environment
- Ability to input clinical data proficiently
Desired Qualifications
- Knowledge of Word and excel preferred
