Job Summary
Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, and nursing home settings across pre and post bereavement phases. Develop social work plans of care, goals, and discharge recommendations for direct admissions and facility residents. Serve as a social work member of the hospice IDG, acting as a liaison between hospice staff and inpatient or nursing home personnel. This role requires New York State MSW licensure, a valid driver's license, and at least one year of healthcare social work experience. You will work within an interdisciplinary team, input clinical data proficiently, and utilize Word and Excel. Based on Fee For Service rates, compensation includes $43/hour for LMSWs or $46/hour for LCSWs, with in-person patient evaluations at $60/visit.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Valid NYS Drivers license and car
- Ability to work within an interdisciplinary team environment
- Ability to input clinical data proficiently
