Job Summary
Provide psychosocial support and social work services to hospice patients and families across inpatient, home care, and nursing home settings. Develop comprehensive plans of care, goals, and discharge recommendations based on patient and family needs. Serve as a liaison between hospice staff and inpatient or nursing home personnel. You will work within an interdisciplinary team environment, input clinical data proficiently, and manage pre- and post-bereavement phases. This role requires New York State MSW licensure, a valid driver's license, and at least one year of healthcare social work experience.
Required Qualifications
- Graduate from the an accredited School of social work.
- Minimum of one year experience in Social Work in a healthcare setting.
- New York State MSW licensure.
- Valid NYS Drivers license and car required.
- Ability to work within an interdisciplinary team environment.
- Ability to input clinical data proficiently.
Desired Qualifications
- Knowledge of Word and excel preferred.
