Job Summary
Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, and nursing home settings across pre, post, and bereavement phases. Develop individualized plans of care, goals, and discharge recommendations based on patient needs for direct admissions and facility residents. Serve as a social work liaison between hospice staff and inpatient or nursing home personnel to ensure coordinated care. This role requires New York State MSW licensure, a valid driver's license, and at least one year of healthcare social work experience. A graduate degree from an accredited school of social work is required, with proficiency in clinical data entry and knowledge of Word and Excel preferred.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Valid NYS Drivers license and car
- Ability to work within an interdisciplinary team environment
- Ability to input clinical data proficiently
