Job Summary
Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, and nursing home settings across pre and post bereavement phases. Serve as a social work member of hospice IDG and liaison between hospice staff and inpatient or nursing home personnel. Develop social work plans of care, goals, and discharge recommendations based on patient and family needs for direct admissions or facility residents. This role requires New York State MSW licensure, a valid driver's license, and at least one year of healthcare social work experience. Compensation includes fee-for-service rates of $43/hour for LMSWs and $46/hour for LCSWs, with visit-based pricing ranging from $40 to $69 depending on service type and specialty.
Required Qualifications
- Graduate from the an accredited School of social work.
- Minimum of one year experience in Social Work in a healthcare setting.
- New York State MSW licensure.
- Valid NYS Drivers license and car required.
- Ability to work within an interdisciplinary team environment.
- Ability to input clinical data proficiently.
Desired Qualifications
- Knowledge of Word and excel preferred.
