Job Summary
Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, and nursing home settings across pre and post bereavement phases. Develop social work plans of care, goals, and discharge recommendations for direct admissions and inpatient facility residents. Serve as a social work member of the hospice IDG and act as a liaison between hospice staff and inpatient or nursing home personnel. This role requires New York State MSW licensure, a valid driver's license, and at least one year of healthcare social work experience. A graduate degree from an accredited school of social work is mandatory, with proficiency in clinical data input and knowledge of Word and Excel preferred.
Required Qualifications
- Graduate from the an accredited School of social work.
- Minimum of one year experience in Social Work in a healthcare setting.
- New York State MSW licensure.
- Valid NYS Drivers license and car required.
- Ability to work within an interdisciplinary team environment.
- Ability to input clinical data proficiently.
Desired Qualifications
- Knowledge of Word and excel preferred.
