Job Summary
Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, and nursing home settings across pre, post, and bereavement phases. Develop individualized social work plans of care, goals, and discharge recommendations based on patient and family needs for direct admissions. Serve as a social work member of the hospice IDG, acting as a liaison between hospice staff and inpatient or nursing home personnel. This role requires New York State MSW licensure, a valid driver's license, and at least one year of healthcare social work experience. Candidates must be able to work within an interdisciplinary team and input clinical data proficiently. Knowledge of Word and Excel is preferred. The position offers competitive fee-for-service rates, including $43/hour for LMSWs and $46/hour for LCSWs, with a focus on quality care within a diverse, not-for-profit health system in the Greater New York area.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Valid NYS Drivers license and car
- Ability to work within an interdisciplinary team environment
- Ability to input clinical data proficiently
