Job Summary
Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, and nursing home settings across pre and post bereavement phases. Develop social work plans of care, goals, and discharge recommendations based on patient needs for direct admissions and facility residents. Serve as a liaison between hospice staff and inpatient or nursing home teams while inputting clinical data proficiently within an interdisciplinary environment. Requires New York State MSW licensure, a valid driver's license, and at least one year of healthcare social work experience.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Valid NYS Drivers license and car
- Ability to work within an interdisciplinary team environment
- Ability to input clinical data proficiently
Desired Qualifications
- Knowledge of Word and excel preferred
