Job Summary
Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, and nursing home settings across pre and post bereavement phases. Develop comprehensive social work plans of care, goals, and discharge recommendations based on patient and family needs for direct admissions. Serve as a social work member of the hospice IDG and act as a liaison between hospice staff and inpatient or nursing home personnel. This role requires New York State MSW licensure, a valid driver's license, and at least one year of healthcare social work experience. Compensation includes fee-for-service rates of $43/hour for LMSW and $46/hour for LCSW, with in-person visits at $60 and telehealth at $40. The position is full-time within a large not-for-profit health system in the Greater New York area, offering a work/life balance and minimal supervision.
Required Qualifications
- Graduate from the an accredited School of social work
- Minimum of one year experience in Social Work in a healthcare setting
- New York State MSW licensure
- Valid NYS Drivers license and car
- Ability to work within an interdisciplinary team environment
- Ability to input clinical data proficiently
