Job Summary
Provide psychosocial support and social work services to hospice patients and families in inpatient, home care, and nursing home settings across pre, during, and post-bereavement phases. Develop comprehensive social work plans of care, goals, and discharge recommendations based on individual patient and family needs. Serve as a liaison between hospice staff and inpatient or nursing home personnel to coordinate interdisciplinary care. This role requires New York State MSW licensure, a driver's license, and at least one year of healthcare social work experience. Compensation is structured on a fee-for-service basis, with rates ranging from $43 to $69 per visit depending on specialty and modality.
Required Qualifications
- Graduate from the an accredited School of social work.
- Minimum of one year experience in Social Work in a healthcare setting.
- New York State MSW licensure.
- Valid NYS Drivers license and car required.
- Ability to work within an interdisciplinary team environment.
- Ability to input clinical data proficiently.
Desired Qualifications
- Knowledge of Word and excel preferred.
