Provide social work services to patients in acute care, maternal child care, emergency department, and home health/hospice settings for patients with acute, chronic, or terminal illness and their families. Responsibilities include identifying psychosocial needs, developing and implementing plans of care, conducting discharge planning, providing crisis intervention, offering patient family education, collaborating with multidisciplinary care teams, and connecting families with community resources.
