Job Summary
Provide social work services to patients in various settings including acute care, maternal child care, emergency department, and home health/hospice. Responsibilities include identifying psychosocial needs, developing and implementing care plans, discharge planning, crisis intervention, patient and family education, and collaborating with a multidisciplinary care team. Requires a Bachelor degree in Social Work, a minimum of one year of experience with a preference for two years in a medical field, and a Licensed Bachelor of Social Work.
