Job Summary
Conduct psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, and crisis intervention for patients and families. Partner with interdisciplinary teams to advocate for social and emotional needs while monitoring health care resources to achieve desired clinical and financial outcomes. Provide resource referrals and assume a leadership role to facilitate patient goals in acute medical or mental healthcare settings. Adhere to the NASW Code of Ethics and Swedish Mission and Vision in daily practice.
Required Qualifications
- Master's Degree in Social Work from an accredited school
- Upon hire: Washington Clinical Independent Social Worker Associate License
- 1 year of experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting
- Upon hire: Washington Clinical Independent Social Worker License
