Job Summary
Conduct medically related social work services supporting the physical, mental, and psychosocial well-being of residents and families. Perform case management for a caseload of 40+ residents, including initial psychosocial assessments, benefit navigation (SSI, Medicaid), and advocacy for legal/social needs. Collaborate with interdisciplinary teams, physicians, and therapists to facilitate admissions, coordinate safe discharge planning with DME and therapy referrals, and provide end-of-life services and bereavement support. Maintain documentation in the EMR and Care Plan sections per regulatory guidelines. Work Monday-Friday 9:30-5:30 with rotational Saturday coverage.
Required Qualifications
- Master Social Worker Degree (MSW) from an accredited University in Social Work
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
- Two years of post-graduate experience, including work with families and children, preferably in a pediatric setting
- Experience or exposure to medically complex children and their families
- Must be able to read, write, speak, and understand the English language fluently
- Must meet or exceed all health standards and requirements for the position as established by NYDOH and all other regulatory agencies
- Must meet the general health requirements set fourth by the policies of this Center which includes a medical and physical examination
- Must be able to push, pull, move, and/or lift a minimum of thirty-five (35) pounds
- Must be able to assist in the evacuation of residents, if necessary
- Monday-Friday 9:30-5:30 w/rotational Saturday
