Job Summary
Counsels and assists residents and families, conducts psychosocial assessments, and develops plans within the Interdisciplinary Care Plan process. Responsibilities include interviewing residents and families, identifying needs, facilitating mobilization of internal and environmental resources, ensuring regulatory documentation, providing information and tours to prospective residents and families, coordinating admissions, and collaborating with staff to connect residents with community resources. The role emphasizes adherence to organizational policies and Judaic values of respect, compassion, advocacy, education, safety, and quality in delivering social services within a skilled nursing and rehab setting.
Required Qualifications
- Hold a current license under the Social Workers, Marriage and Family Therapists and Professional Counselors Act; or hold a bachelor’s degree (B.A.) or a master’s degree (M.S.W.) from a four-year college or accredited school/university in Social Work or social welfare.
