Job Summary

Conduct Care Act 2014 assessments, reviews, and Mental Capacity Assessments for adults transitioning from hospital to the community. Develop person-centred support plans, manage safeguarding concerns, and assess risks to ensure safe discharges. Undertake face-to-face visits across Solihull and surrounding areas while maintaining accurate statutory case recordings. Collaborate with health professionals, hospital teams, and carers to facilitate independent living. Requires full UK driving licence, Social Work England registration, and significant adult social care experience.

Required Qualifications

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW
  • Registration: Must be registered with Social Work England
  • Experience: Significant post-qualified experience within Adult Social Care, including hospital discharge, assessment, and support planning
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance
  • Assessment Skills: Proven ability to complete high-quality Care Act assessments, Mental Capacity Assessments, and outcome-focused support plans
  • Safeguarding Experience: Experience managing safeguarding concerns and undertaking risk assessments within adult social care practice
  • Skills: Excellent communication, assessment, organisation, and time management skills with the ability to work autonomously and manage competing priorities
  • Driving Licence: Full UK driving licence and access to a vehicle
