Job Summary
Instruct, treat, observe, and evaluate patients with significant social and emotional problems affecting their health status in their homes. Perform psychosocial assessments, counsel families on illness coping, and implement physician Plan of Care provisions including community referrals. Assess pre-bereavement needs and apply infection control standards during client care activities. Complete timely documentation, maintain required licenses and certifications, and participate in mandatory continuing education. Work Monday through Friday with alternating weekends, lifting up to 50 pounds and standing for extended periods.
Required Qualifications
- Master's Degree from a school of Social Work accredited by the Council on Social Work Education.
- Clear and Active Professional License to practice in the state of Florida.
- 1-2 years of social work experience in a health care setting.
- Knowledge of AHCA Regulations, Medicare COP's Guidelines required.
- Must have knowledge of computer office/clinical software.
- Must be able to read, write and understand the English language.
- Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.
Desired Qualifications
- Home care experience preferred.
- Knowledge of public health administration and/or social work consultation and/or supervision preferred.
